Lifetime Weight Management Introduction to the program. A hundred years ago life was very different from today in many ways. A greater percentage of people worked harder physically, and the stresses of attempting to cram 90 minutes into every waking hour was the exception not the rule. The most important difference however was the way in which we ate. No single factor has changed so dramatically and so rapidly as the dietary intake of the average person. At the turn of the century Americans mostly ate natural home-cooked meals, not commercial packaged foods filled with chemicals and lifeless calories. Today, for many of us, it's just the opposite. What society offers is margarine or some other fat substitute for our bread instead of real butter, our fruits and vegetables are sprayed with chemicals of a wide variety, and our meats are injected with chemicals and powerful metabolic altering hormones. But the most important change of all perhaps, is in our consumption of sugar. In our grandparents' day the average person consumed between 3 and 5 pounds of sugar per year. Today the average individual consumes his or her won body weight in sugar during the same period! This powerful and potentially dangerous substance, consumed in such enormous amounts, is wreaking havoc with the health of each of us, from small children to senior citizens. Among the many detrimental effects of this over-consumption are glucose metabolism disorders, such as carbohydrate intolerance, hypoglycemia, and diabetes. Glucose metabolism disorders occur when the body cannot properly metabolize sugars for fuel. Chronic conditions like hypoglycemia are so common that they are often overlooked by diagnosing health practitioners. Adult onset diabetes, the most rapidly spreading epidemic in modern times, costs the United States 85 billion dollars annually - and that figure is expected to double before the passing of another decade. One of the more obvious side effects of too little exercise and too many calories, or more accurately "wrong calories", is obesity. There are more overweight people in America than any other nation on earth. Is it because we live in a land of plenty and simply eat too much? For many this is the case. A reduction in exercise and an increase in caloric consumption will produce moderate weight gain in many individuals. That "extra" 10 or 15 pounds is frequently due to excess availability and poor food choices. But what about the millions of people who are truly obese? Is this simply a product of eating too much? No, it is not. Years of research, from athletes to the chronically overweight, shows that weight problems go beyond the simple concept of over-indulgence. In a short while we will discuss a complicated metabolic process that can lead to far more than extra unwanted pounds. Reasons you need to normalize your weight. Being overweight has greater implications than detracting from one's appearance. Excess body fat, in combination with other factors such as lack of exercise, can be directly linked or at least contribute to a host of health problems, many of which lead to chronic degenerative disease. In short, being overweight is downright dangerous! It is estimated that 35% of men over thirty and 50% of women over forty are 20 pounds or more over their ideal weight. This can lead to a variety of direct linked diseases such as hypertension, heart disease, stroke and diabetes. Further, obesity has been implicated in the increased incidence of many other health problems including kidney disease, cirrhosis of the liver, gastric and duodenal ulcers, arthritis, gall bladder disease, urinary infection, complications during pregnancy, low back pain, edema, hyperlipidemia, hernia, varicose veins, depression and suicide. Still further, cancers of the colon, rectum, prostate, breast, uterus and ovaries are significantly greater in persons 20% above their ideal body weight. Overall an individual with just a 20% above their ideal body weight has a 350% greater mortality risk! In short, excess amounts of the wrong types of fat in the absence of exercise can lead to many acute and chronic health problems - problems that might have been avoided if properly managed. Before we begin our discussion of the actual management of body fat, we need to understand why some of us can eat constantly and remain this as a rail, while others seem to gain 2 pounds just "breathing the fumes" of a summering casserole. The answer lies in your metabolism. Picture your body as an engine with a more or less fixed idling speed. This idling or resting rate is called your basal metabolic rate (BMR), which varies from person to person. Some people have a higher or faster BMR than others and are thereby able to consume more calories without storing them as excess fat. Others have a slower resting BMR during times of greater caloric intake, thus compensating for the excess calories without gaining weight. Calorie Sensitive Individuals who have a slow BMR and who don't exercise enough to utilize the fuel from the foods they eat are known as "calorie sensitive" - meaning that their caloric intake exceeds what their body is able to burn. These individuals respond well to a dietary program that restricts their calorie intake to match their calorie output. These individuals usually lose weight rather quickly upon reducing calories and generally are not more than 10 to 25 pounds overweight. The weight gain usually occurs after the age of 35 due to a slowing of the metabolism for which the dietary intake has not been altered or adjusted accordingly. If these symptoms describe you, you are likely a calorie sensitive individual. An easy calorie sensitive self-evaluation test has been provided for you in this booklet. Which are You? Carbohydrate Intolerant During 25 five years of practice in the nutrition field, I have found that more heavily overweight people (those with more than 30 pounds excess body fat) respond poorly to low calorie diets. This is because they are generally not calorie sensitive but rather "carbohydrate intolerant". Individuals who are carbohydrate intolerant usually lose weight with great difficulty, even on low calorie diets. It is not uncommon for these people to go on a 1500 calorie diet and in spite of following it perfectly, not lose more than 1 or 2 pounds per month - or worse yet, lose no weight at all for months at a time! How can you tell if you are carbohydrate intolerant? Ask yourself these questions: Are you overweight despite the fact that you don't frequently overeat? Have you stuck with calorie restricted diets faithfully, and still only lost little weight or none at all? Do you have abnormal obsessions with foods? A craving for sweets, pastas and breads? If you can answer yes to these questions you are very likely carbohydrate intolerant. An easy carbohydrate intolerance self-evaluation test has been provided for you on page 6 of this booklet. Self Evaluation Tests In order to properly evaluate your metabolic type (calorie or carbohydrate sensitive) take the following self tests. Test for Calorie Sensitivity If you answer yes to the majority of the following statements you are likely consuming more calories than you are burning and need to follow our calorie restriction program outlined on page 13. Take the Test 1. You had a normal body weight when younger but slowly gained weight after age 30. 2. You are presently overweight but by less than 25 pounds. 3. You have a normal appetite - get hungry at mealtimes. 4. You have few, if any food cravings. 5. You have maintained the same basic eating habits all your life. 6. You eat three meals per day. 7. You have gained a certain amount of extra body weight but seem to have tapered off (not continued to steadily gain more and more weight). 8.You have few or none of the symptoms associated with poor carbohydrate metabolism ( see the following list). Test for Carbohydrate Intolerance If you can answer yes to the majority of these statements you are likely carbohydrate intolerant and should follow our carbohydrate restriction program. 1. You are more than 25 pounds overweight. 2. You have had a tendency to be overweight all your adult life. 3. You have been overweight since you were younger. 4. You have a poor appetite and often skip meals. 5. You prefer not to eat in the morning. 6. You have food cravings that temporarily go away when starchy or sugary foods are eaten. 7. There are foods that you feel you absolutely could not do without. 8. Most or all of the symptoms associated with carbohydrate intolerance apply to you. (See list of symptoms). Symptoms related to carbohydrate intolerance For many thousands of people mere calorie excess alone does not account for their weight problem. They have further metabolic disturbances which revolve around two problems, hyper- insulinism and insulin resistance. When you suffer from either hyper-insulin or insulin resistance (the main causes of carbohydrate intolerance) they can lead not only to excessive weight gain but also to other blood sugar related conditions such as hypoglycemia, adult onset diabetes and a far greater incidence of heart disease, stroke and hypertension. Earlier signs of insulin problems leading to these and other health conditions are listed below. While many of the following symptoms may be interchanged with or applied to many other health conditions, the more of them you experience on a regular basis the more likely it is that you have serious insulin problems which are causing your obesity. 1. Nervousness 2. Irritability 3. Fatigue & Exhaustion 4. Faintness, dizziness 5. Depression 6. Drowsiness, especially 7. Headaches 8. Digestive disturbances with no obvious cause 9. Forgetfulness 10. Understanding the basic concepts of weight management For most people who are overweight, a change in eating habits is required in order to control their problem. It is for this reason that many get on the treadmill of roller coaster weight, repeatedly dieting to lose weight only to gain it back again. Whether you are calorie or carbohydrate sensitive, a permanent change in eating and other lifestyle habits is necessary. One of the biggest frustrations in attempting a weight loss program is knowing which way to go. What program is best? Through our previous questionnaires you can determine your body type and begin to follow a program that is best suited to your body chemistry. While each program will require modifications to your present eating habits, once you know what your body chemistry is, you can achieve your desired results much faster. Another major factor in your weight loss success is your mental attitude. When we eat foods we know are not good for us, we frequently feel guilty afterwards. This creates stress within the body and causes us to lose more self-esteem, which in turn makes us eat more as a way of comforting ourselves. Meals for all Occasions Recipes for healthy eating - no more boring meals When we contemplate going on a diet one of the first things that comes to mind is boring meals with celery sticks, salads with no dressings and entree's with no flavor. No wonder people don't stay on a diet! Dieting does not have to consist of boring, dull or tasteless meals. If you are calorie sensitive you can eat the same basic diet as before with an emphasis on reducing fat intake, since fat has more than twice as many calories as any other food. This will allow you to partake in every course while keeping your carbohydrate grams under control. Soups are also no problem. Make a vegetable and meat based soup, just remember not to add startch as a thickener and leave starchy vegetables such as potatoes and pasta out. Cream soups are also acceptable. Main courses present no problem. You can serve a meat, fish or fowl entree, combined with a steamed vegetable or a garden salad from the vegetables listed above. The big question, how can you enjoy an occasional sweet tasting dessert and not abuse your carbohydrates? While this is certainly more difficult, there are some desserts, especially if you make them yourself and know exactly what is in them, that you can enjoy. Since the dessert category always seems to be the most challenging, we have included a few recipes and suggestions that are favorites of the author for your consideration. Low Calorie Jell-O Follow the instructions to make low calorie Jell-O. You can vary by adding homemade whipping cream (not the sugar- filled variety from a can) You can also add a variety of nuts and even strawberries. For those who are carbohydrate intolerant, these delicious desserts are suggested (remember it is the carbohydrate calories we want to avoid, rather than the fat calories): Fresh Strawberries with Fresh Cream An old English favorite! Fresh- cut strawberries (almost no carbohydrates) with thick fresh cream and a little artificial sweetener makes an excellent dessert that satisfies the sweet tooth and makes a refreshing warm weather treat. Chocolate Mousse 6 ounces semisweet chocolate 1/2 cup (1 stick) unsalted butter 3 eggs, separated 2 tablespoons sugar 1 cup heavy cream 1 teaspoon vanilla extract Melt chocolate with butter in a double broiler. After cooling add the egg yolks and stir well. Beat egg whites to soft peaks, then while beating constantly, add egg whites slowly to the chocolate mixture.
